Quite obviously, jdcsampl.tex is the sample file and around the auxiliary files left over after TeX-ing it.

The authors addresses and affiliation are typed "as is" at the end of the
manuscript without invoking AMS macros. The bibliography also is
referred to by numbers in square brackets and typed "as is" (not using
\bibitem or similar devices).
In short, you have to be very much motivated if you wish to see your paper on screen exactly as it will appear in print, but in princple this can be done.
